Description : Pursuant to a congressional request, GAO reviewed: (1) how the Office of Management and Budget (OMB) planned to increase federal agencies'' awareness of unclaimed properties held by the District of Columbia and other state and local entities; and (2) the nature and value of those properties. GAO found that: (1) the District of Columbia''s Unclaimed Property Division annually reported unclaimed properties by listing the owners'' names and addresses in local newspapers to notify them of those unclaimed properties; (2) only 45 of the 8,740 unclaimed properties the District listed in 1989 belonged to federal agencies; (3) 41 of those properties had a total value of $30,669, less than 1 percent of the value of all 1989 unclaimed properties; (4) 3 federal agencies reclaimed $3,042 of the $30,699 after being notified, but agencies did not reclaim the remaining $27,657 in property; and (5) 82 percent of the unclaimed federal properties belonged to 6 federal agencies that generally did not know that those properties were being held.

Description : The Matanuska-Susitna Valley is in the northeastern part of the Cook Inlet Basin, Alaska, an area experiencing rapid population growth and development proximal to many lakes. Here water commonly flows between lakes and ground water, indicating interrelation between water quantity and quality. Thus concerns exist that poorer quality ground water may degrade local lake ecosystems. This concern has led to water-quality sampling in cooperation with the Alaska Department of Environmental Conservation and the Matanuska-Susitna Borough. A map showing the estimated altitude of the water table illustrates potential ground-water flow directions and areas where ground- and surface-water exchanges and interactions might occur. Water quality measured in selected wells and lakes indicates some differences between ground water and surface water. ''The temporal and spatial scarcity of ground-water-level and water-quality data limits the analysis of flow direction and water quality. Regionally, the water-table map indicates that ground water in the eastern and southern parts of the study area flows southerly. In the northcentral area, ground water flows predominately westerly then southerly. Although ground and surface water in most areas of the Matanuska-Susitna Valley are interconnected, they are chemically different. Analyses of the few water-quality samples collected in the area indicate that dissolved nitrite plus nitrate and orthophosphorus concentrations are higher in ground water than in surface water.

GAO found that: (1) IRS plans to spend about $150 million on the Communications Replacement System through 1993; (2) IRS estimates that it will obligate less than it originally planned for the system through 1988; (3) the budget estimate changed because a successful bid protest forced IRS to make award to a higher-priced bidder; and (4) IRS expects to have the system installed about 1 year later than originally scheduled. GAO also found that: (1) IRS estimates that it will spend billions of dollars over the lifetime of its redesigned tax system; (2) for FY 1982 through 1988, IRS estimated obligations of $83.6 million for the redesign, but actually obligated only about $71 million; and (3) the redesign estimates changed because IRS repeatedly changed its redesign strategy.

Description : In response to a congressional request, GAO reviewed the military services'' fiscal year (FY) 1988 budget requests for ammunition items and the Army''s request for ammunition production base support. GAO noted that the budget request totalled $3.6 billion for ammunition items and $168.5 million for ammunition production facilities. GAO believes that the Army adequately justified its request for the ammunition production base; however, GAO estimated that the services inadequately justified $577.6 million of the $3.6 billion request for ammunition items. GAO found that the Army overstated its request by $320.9 million because: (1) eight developmental items will not be ready for procurement; (2) two items have overly optimistic production schedules; (3) four items have unresolved component problems; (4) two items have excessively long procurement lead times; (5) it will not need the total program quantity for two items in FY 1988; and (6) four items have larger quantities than needed. GAO found that the Navy overstated its $566.3 million for ammunition by $63.1 million due to: (1) production problems for three items; (2) prematurely programmed procurements; (3) overstated unit costs; (4) a possible replacement with lower cost, alternative components; and (5) items that exceed inventory requirements. GAO found that the Air Force overstated its $643.4 million request for ammunition by $189.1 million because: (1) it will not require certain item quantities to meet 1988 delivery schedules; and (2) lead times are excessive. GAO also found that the Marine Corps overstated its $410.9 million request for ammunition by $4.5 million because a new fuze will not be ready in time for procurement in FY 1988.

Description : GAO reviewed the effectiveness of the Social Security Administration''s (SSA) process for crediting individuals'' earnings to their individual accounts and the effect of uncredited or erroneously credited earnings on individuals'' benefits and eligibility and on Social Security trust funds. GAO found that: (1) SSA consistently reported less in earnings than the Internal Revenue Service (IRS), and the two agencies have not worked well together to resolve differences in employers'' earning reports; (2) SSA contact with employers to resolve such differences resulted in recording about an additional $3.6 billion in earnings for 700,000 employees, although half of employers contacted did not provide information; (3) neither SSA nor IRS compiled sufficient data to identify the causes of differences and actions necessary to prevent or reduce future occurrences; (4) three of five individuals with uncredited earnings faced possible loss of about $17 a month in benefits; (5) SSA plans to resolve some backlogged uncredited earnings reports did not encompass all such reports, or address employers who did not respond to information requests, or ensure that employers saved employees'' earning records; and (6) unless it can certify recorded earnings for previous tax years, SSA may not be entitled to a portion of tax revenues received based on IRS records.

Description : GAO's work has shown that the Department of Homeland Security (DHS), through its component agencies, particularly the Coast Guard and U.S. Customs and Border Protection (CBP), have made substantial progress in implementing various programs that, collectively, have improved maritime security. In general, GAO's work on maritime security programs falls under four areas: (1) security planning, (2) port facility and vessel security, (3) maritime domain awareness and information sharing, and (4) international supply chain security. DHS has, among other things, developed various maritime security programs and strategies and has implemented and exercised security plans. For example, the Coast Guard has developed Area Maritime Security Plans around the country to identify and coordinate Coast Guard procedures related to prevention, protection, and security response at domestic ports. In addition, to enhance the security of U.S. ports, the Coast Guard has implemented programs to conduct annual inspections of port facilities. To enhance the security of vessels, both CBP and the Coast Guard receive and screen advance information on commercial vessels and their crews before they arrive at U.S. ports and prepare risk assessments based on this information. Further, DHS and its component agencies have increased maritime domain awareness and have taken steps to better share information by improving risk management and implementing a vessel tracking system, among other things. For example, in July 2011, CBP developed the Small Vessel Reporting System to better track small boats arriving from foreign locations and deployed this system to eight field locations. DHS and its component agencies have also taken actions to improve international supply chain security, including developing new technologies to detect contraband, implementing programs to inspect U.S.-bound cargo at foreign ports, and establishing partnerships with the trade industry community and foreign governments.

Description : Pursuant to a congressional request, GAO investigated allegations that federal agencies aided the immigration of Nazi war criminals and collaborators and concealed their backgrounds once they were admitted. GAO determined whether: (1) there were any federal programs specifically designed to aid Nazis and collaborators; and (2) federal intelligence agencies protected a certain prominent Gestapo official. <br /> GAO found that, after World War II, U.S. intelligence agencies were required to change from an emphasis on wartime counterintelligence to an emphasis on gathering information about the Soviet Union and the worldwide Communist movement. The intelligence community was not well prepared for this change because: (1) wartime intelligence units had been disbanded and many experienced personnel left the military; and (2) those experienced personnel who remained in the intelligence community were trained in counterintelligence rather than in covert intelligence operations. GAO found that: (1) U.S. intelligence agencies knowingly employed alleged Nazis and Axis collaborators in order to obtain information about Soviet intentions and capabilities because Germany was the only country whose intelligence apparatus was targeted at Soviet activities; (2) in at least five instances, U.S. intelligence agencies provided immigration assistance to individuals with known undesirable or questionable backgrounds; and (3) it could not determine the total number of such individuals who were given immigration assistance. However, GAO found no evidence of any specific program to provide immigration assistance to potentially undesirable individuals and noted there were several instances in which the use of Nazis or collaborators was rejected because of their pasts. In addition, GAO reviewed a Department of Justice report on the protection and employment of a prominent Gestapo official and found no discrepancies with information available to GAO.

Description : GAO assessed the Department of Labor''s random audit program which was initiated to improve the accuracy of benefit payments under the unemployment insurance system. In each participating state, a random sample of about eight unemployment insurance payments is selected for detailed weekly investigations and this provides the basis for estimating the extent, type, and cause of overpayments. GAO believes that the program is conceptually sound and should, if properly implemented, provide needed information. However, comparisons between states may not be meaningful, statistically reliable projections of a national overpayment rate cannot be made until all states have implemented the random audit, not all unemployment insurance claims are covered by the random audit, and overpayment rates tend to be understated. The results of the random audit program have generally confirmed that high overpayment rates exist. The principal identified cause for these overpayments is that claimants are not actively seeking work. In addition, there are limitations in the states'' systems for detecting, preventing, and recovering overpayments. GAO believes that overpayments can be greatly reduced if Labor and the states effectively use the random audit information. In several states, the program is contributing to improved claims processing and overpayment control. However, others have not made substantial changes to improve their programs, principally because of Labor''s initial passive role in seeking corrective actions. Concern exists as to whether the federal monitoring of the program in each state should be strengthened and whether the program should be expanded to include additional types of unemployment insurance claims.

Description : In response to a congressional request, GAO reviewed the black lung workers'' compensation and medical expense payment systems administered by the Department of Labor''s Office of Workers'' Compensation Programs (OWCP). GAO estimated that, through 1982, Labor had incorrectly paid 26 percent of the beneficiaries in the GAO sample, resulting in overpayments and underpayments totaling $65 million. GAO also estimated that an amount equaling $24 million of these errors had been corrected by the time that GAO initiated its case file review. In addition, Labor also identified other overpayments which totaled about $5 million. Beneficiaries were inappropriately receiving more than one black lung benefit from Labor, the Social Security Administration (SSA), or from a state workers'' compensation program. According to the responsible officials, many payment errors were caused by the work loads created by amendments to the original black lung legislation. GAO identified several problem areas affecting quality control and recordkeeping which contributed to the payment errors. Labor has recognized many of these problems and has initiated actions to reduce future errors. GAO also found that problems in medical payments identified during an inspector general''s review still exist and that Labor has not used fee schedules to ensure that medical payments are reasonable.

FDA awarded grants totaling about $5 million to four pediatric device consortia in these 2 years. Of the device projects assisted by the consortia, many projects were in early stages of development, such as the creation of a prototype and the preclinical testing of that prototype. Activities performed by the consortia included regularly scheduled forums where innovators could discuss new device ideas with a group of experts.

Description : This report discusses the briefing we gave on June 8, 2011, as well as subsequent comments from the Internal Revenue Service (IRS). We gave this briefing in response to Congressional request that we assess IRS''s planning to implement its responsibilities under the Patient Protection and Affordable Care Act (PPACA). The report (1) describes IRS''s PPACA responsibilities and effective dates and (2) assesses the extent to which IRS, in planning PPACA implementation, is following leading practices in four areas--developing an overall management structure (including goals and performance measures), estimating and tracking costs, assuring compliance with the new law while minimizing burden, and managing risk. In summary, IRS has responsibilities in the implementation of 47 PPACA provisions with effective dates through 2018. In planning to implement these provisions, IRS has generally followed leading practices. Top leadership has been involved; cost estimates for information technology projects have specified ground rules and assumptions, data sources, and supporting calculations; work has started on compliance controls; and risks are being identified and analyzed at the individual project level. However, IRS could improve aspects of its planning, particularly at an agencywide or strategic level. IRS defines strategic-level goals and project plans in multiple documents without integrating the goals or plans, IRS has no timeline for developing performance measures and collecting associated data, a cost estimate for all of the PPACA program has not been provided, and the risk management framework does not assure that all risks, especially strategic-level risks, are identified and analyzed. While implementation for some provisions is years away, making improvements to the planning process now would reduce risks and might minimize future problems. Hence, we are recommending that the Commissioner of Internal Revenue take the following four actions: (1) define program goals and develop a project plan in one document that effectively integrates all aspects of the program; (2) document a schedule for developing performance measures that link to program goals; (3) develop a more complete cost estimate that is consistent with the GAO Cost Estimating Guide; and (4) modify and document IRS''s risk management approach to have more assurance that all risks, including strategic-level risks for the program, are identified and analyzed, and that mitigation options are assessed.

Description : Of the estimated 1.1 million Americans living with HIV, not all are aware of their HIV-positive status. Timely testing of HIV-positive individuals is important to improve health outcomes and to slow the disease's transmission. It is also important that individuals have access to HIV care after being diagnosed, but not all diagnosed individuals are receiving such care. The Centers for Disease Control and Prevention (CDC) provides grants to state and local health departments for HIV prevention and collects data on HIV. In 2006, CDC recommended routine HIV testing for all individuals ages 13-64. The Health Resources and Services Administration (HRSA) provides grants to states and localities for HIV care and services. GAO was asked to examine issues related to identifying individuals with HIV and connecting them to care. This report examines: 1) CDC and HRSA's coordination on HIV activities and steps they have taken to encourage routine HIV testing; 2) implementation of routine HIV testing by select state and local health departments; 3) available information on CDC funding for HIV testing; and 4) available data on the number of HIV-positive individuals not receiving care for HIV. GAO reviewed reports and agency documents and analyzed CDC, HRSA, and national survey data. GAO interviewed federal officials, officials from nine state and five local health departments chosen by geographic location and number of HIV cases, and others knowledgeable about HIV.

GAO also found that: (1) the largest guarantor of student loans was also the biggest payer of default claims to lenders; (2) the guarantor''s share of default claims paid to lenders increased as its loan volume remained steady and subsequently decreased; (3) in July 1988, the guarantor stopped guaranteeing loans because the majority of its loans were for students attending proprietary schools, and it had $8.8 billion in outstanding guaranteed loans; and (4) the Department of Education reinsured guaranty agencies for 80 to 100 percent of loans, depending on their default rates.

Description : Pursuant to a legislative requirement, GAO reviewed the federal school construction program for school districts affected by federal activities. GAO found that: (1) in 1988, appropriation shortfalls had created a backlog of 178 eligible unfunded construction projects in school districts; (2) total estimated federal payments for those projects could be about $216 million; (3) information on the number of federally connected children and the estimated costs of many eligible projects that remain unfunded from year to year is often outdated; (4) in 1989, school districts that submitted 20 of the 58 projects that GAO studied had already completed the projects; (5) school district officials that submitted 8 of the 20 completed projects said they did not need federal construction assistance; (6) because most project requests are at least 12 years old, the project priority scores may be outdated and invalid and the school districts may subsequently complete their projects without federal assistance; (7) since the Department of Education fails to reevaluate project priority scores, it cannot provide Congress with an accurate ranking of federally impacted schools with current school construction needs; and (8) due to increased construction costs, project payments to schools receive a smaller share of total construction costs than they would if they were funded promptly.

Description : Pursuant to a congressional request, GAO evaluated conflicting cost estimates that the United States and Israel prepared for the Lavi fighter aircraft, which Israel is developing to replace its older fighter aircraft. GAO found that: (1) Israel financed 90 percent of the development and initial production of the Lavi with U.S. foreign military sales (FMS) loans that Congress forgave; (2) the U.S. aircraft industry is heavily involved in the Lavi program; and (3) the Department of Defense (DOD) estimated that the Lavi would cost about $22.1 million per aircraft, or 52 percent more than the Israeli estimate of $14.5 million per aircraft. GAO adjusted the Israeli estimate to make it comparable to the DOD estimate and found that: (1) the Israeli-estimated hourly labor rates and other costs appeared low, while the DOD estimate for labor rates was high; (2) DOD overestimated the aircraft''s weight; (3) DOD improperly adjusted its calculations for labor hours; (4) DOD overestimated the cost of materials; and (5) the DOD estimate could have been high by as much as $4.3 million. In addition, GAO found that, even using the lowest estimate, the Lavi''s cost will: (1) exceed an Israeli-imposed annual spending limit for the aircraft; and (2) cause Israel to spend an increasing portion of its defense budget on the Lavi and damage its ability to meet its other defense needs.

Description : Strain energy release rates were computed along straight delamination fronts of Double Cantilever Beam, End-Notched Flexure and Single Leg Bending specimens using the Virtual Crack Closure Technique (VCCT). The results were based on finite element analyses using ABAQUS# and ANSYS# and were calculated from the finite element results using the same post-processing routine to assure a consistent procedure. Mixed-mode strain energy release rates obtained from post-processing finite element results were in good agreement for all element types used and all specimens modeled. Compared to previous studies, the models made of solid twenty-node hexahedral elements and solid eight-node incompatible mode elements yielded excellent results. For both codes, models made of standard brick elements and elements with reduced integration did not correctly capture the distribution of the energy release rate across the width of the specimens for the models chosen. The results suggested that element types with similar formulation yield matching results independent of the finite element software used. For comparison, mixed-mode strain energy release rates were also calculated within ABAQUS#/Standard using the VCCT for ABAQUS# add on. For all specimens modeled, mixed-mode strain energy release rates obtained from ABAQUS# finite element results using post-processing were almost identical to results calculated using the VCCT for ABAQUS# add on.

Description : The previously developed Ko closed-form aging theory has been reformulated into a more compact mathematical form for easier application. A new equivalent loading theory and empirical loading theories have also been developed and incorporated into the revised Ko aging theory for the prediction of a safe operational life of airborne failure-critical structural components. The new set of aging and loading theories were applied to predict the safe number of flights for the B-52B aircraft to carry a launch vehicle, the structural life of critical components consumed by load excursion to proof load value, and the ground-sitting life of B-52B pylon failure-critical structural components. A special life prediction method was developed for the preflight predictions of operational life of failure-critical structural components of the B-52H pylon system, for which no flight data are available.

Description : The differential equation of Chaplygin's jet problem is utilized to give a systematic development of particular solutions of the hodograph flow equations, which extends the treatment of Chaplygin into the supersonic range and completes the set of particular solutions. The particular solutions serve to place on a reasonable basis the use of velocity correction formulas for the comparison of incompressible and compressible flows. It is shown that the geometric-mean type of velocity correction formula introduced in part I has significance as an over-all type of approximation in the subsonic range. A brief review of general conditions limiting the potential flow of an adiabatic compressible fluid is given and application is made to the particular solutions, yielding conditions for the existence of singular loci in the supersonic range. The combining of particular solutions in accordance with prescribed boundary flow conditions is not treated in the present paper.

Description : GAO reviewed the Agency for International Development''s (AID) progress in assisting Egypt in developing its economy, including: (1) the conditions and constraints of establishing and administering the Economic Support Fund (ESF); and (2) the advantages and disadvantages of the current mode of providing ESF assistance and the possible alternative ways of disbursing this assistance. GAO found that: (1) economic development progress has been somewhat limited by program conditions over which AID had little control; and (2) some actions taken to support the political purpose of the program may not be of optimal value in working toward the longer term economic development goal. GAO found that AID had difficulty influencinge Egypt to quicken its pace in making economic policy reforms because: (1) AID lacked overall program funding leverage; and (2) Egypt was concerned that moving too quickly might be politically destabilizing. GAO also found that the large size of the project portfolio has: (1) made it difficult to administer the program; (2) challenged Egypt''s ability to effectively absorb large amounts of assistance; and (3) resulted in a large amount of obligated but undisbursed funds. GAO noted that AID could use nonproject program approaches such as cash transfers, sector grants, and commodity imports as alternative modes for some of the project assistance, but AID needed to objectively and thoroughly assess the various modes to determine the best mix for accomplishing program goals in Egypt.

Description : Pursuant to a congressional request, GAO reviewed the Agency for International Development''s (AID) congressional notification procedures for reprogramming foreign assistance funds to determine whether changes in the process would reduce the administrative burden without unduly limiting congressional oversight. GAO found that: (1) in 1987, AID submitted 700 notifications to Congress, representing 1,171 reprogramming actions; (2) reducing the submissions would be difficult because of widely diversified congressional interests; (3) AID could achieve some reductions without significantly limiting congressional oversight by eliminating notifications for actions that did not increase project funding, raising the reporting threshold, or adopting a dollar-value reporting threshold; and (4) AID could achieve greater reductions if Congress required it to justify its programs and submit notifications of only those reprogramming changes affecting country spending levels.

Description : The Department of Defense (DOD) carried out military herbicide operations in South Vietnam from 1962 to 1971. Herbicide Orange, the most widely used herbicide, contains a contaminant, dioxin, that is highly toxic. Its effects on laboratory test animals have been studied; however, long-term health effects on humans remain largely unknown. In late 1977, Veterans Administration (VA) regional offices began receiving compensation claims from veterans who felt that some of their medical problems were caused by exposure to herbicides in Vietnam. Vietnam veterans also began contacting VA health care facilities to express concerns over possible herbicide exposure. By September 1978, about 600 veterans had been examined at VA health care facilities and about 450 had submitted claims to regional offices. VA has been hampered by the lack of information on long-term health effects of herbicides, and VA has established an interagency advisory group to assist in evaluating the medical aspects of herbicide exposure. Since there are no specific records on herbicide exposure, VA is having difficulty identifying veterans who were exposed to herbicides. DOD officials believe that those most likely to have been exposed were herbicide handlers and aircraft crews flying herbicide missions. VA health care facilities have been instructed to examine any veteran concerned about herbicide exposure, and VA regional offices have been instructed to evaluate herbicide-related claims. Not all veterans submitting claims to regional offices are being referred to VA health care facilities for examination, however, and VA regional offices have not been instructed to obtain information from military records concerning the likelihood of an individual veteran''s exposure to herbicides.

GAO found that: (1) although Department of Defense standards and key Army operational requirements prevent the Army from purchasing commercial trucks to meet its tactical truck requirements, it is making extensive use of commercial components in the tactical trucks it purchases; (2) of the three tactical truck systems reviewed, two systems required significant modifications and integration of commercial and military components to meet Army requirements; (3) all three Army contractors have used commercial trucks as baselines for their systems; (4) the operational requirements that prevent the Army from using baseline commercial trucks include tactical mobility, deployability, transportability, and survivability; (5) tactical truck contractors must meet rigorous testing standards, develop detailed technical manuals, use standard parts in the Army''s inventory system, and adopt military quality standards that are not found in the commercial market place; and (6) although the Army cannot meet its tactical requirements using commercial trucks, it may be able to increase its use of commercial vehicles to meet its non-tactical requirements.

GAO found that AID: (1) relied on cash transfers to encourage economic policy reforms and to address balance-of-payments problems; (2) did not establish sufficient criteria to guide its policy reform efforts; (3) has had mixed success in encouraging policy reforms; (4) encountered problems in its efforts to implement separate accounting for cash transfers, since recipients continued commingling program funds with foreign exchange accounts; (5) failed to require separate accounting for certain ESF grants and projects because it did not consider them cash transfers; and (6) did not verify that recipients complied with its requirement to keep records on separate account disbursement or ensure that they used the funds for authorized purposes.
